Pride Of Sri Lanka - Rumassala @ Unawatuna,Galle

Rumassala Hill was recognized as Buono Vista, this name has a times past  towards theregal period, in spite of origin it is certainly a precise report of the place they chose to name, for the peak of Rumassala mountain offers what are debatably the finest coastal views all along the southern coastline of Sri Lanka. There is a major magnetic anomaly near Unawatuna, which Arthur C. Clarke attributes to a meteorite strike, and it is said that satellites lose their orbits with unusual frequency overhead. If you are in sri lanka take the time to visit Rumassala. Access is via a road opposite the cement factory on the main Galle-Matara road.

  •  E.g.-(Yes, Yes, No, Yes, No, Yes, No, Yes),(42, 63, 96, 74, 56, 86),(111192, 111234)
  • None of the above data sets have any meaning until they are given a CONTEXT and PROCESSED into a useable form
Information are data that has been processed within  a context to give it meaning OR data that has been processed into a form that gives it meaning.

yellow oleander poisoning

Plant poisonings are especially common among South Asian countries like India and
Sri Lanka.  Yellow oleander (ThevetiaPeruviana), an ornamental tree common
throughout the tropics is the source of majority of plant poisonings in these
regions.  Ingestion of oleander seeds, which contains highly toxic glycosides
including thevetins A and B and neriifolin, hampers the cardiac muscle and
autonomic nervous system resulting in cardiac dysrthythmias, sinus and AV node block, bradycardia, vomiting diarrhoea, hyperkalemia etc
Treatment for yellowoleander poisoning includes resuscitation, antidotes, gastric decontamination,
activated charcoal, atropine, cardiac pacing and supportive care
Gastric decontamination is consistently used throughout South Asia as a means of
decontamination. The different methods of gastric decontamination include forced
emesis and gastric lavage. Activated charcoal works by adsorbing poison and in
reducing further absorption.Decontamination techniques for yellow oleander poisoning include forced emesis, gastric lavage and single or multiple doses of activated charcoal.

Malignant Hyperthermia

This is quote article for educational purpose..
    * Author: James W Chapin, MD; Chief Editor: John Geibel, MD, DSc,
Malignant hyperthermia (MH) is a life-threatening clinical syndrome of hypermetabolism involving the skeletal muscle. It is triggered in susceptible individuals primarily by the volatile inhalational anesthetic agents and the muscle relaxant succinylcholine, though other drugs have also been implicated as potential triggers. MH is not an allergy but an inherited disorder that is found both in humans and in swine.

In persons susceptible to MH, the ryanodine receptor in skeletal muscle is abnormal,[2] and this abnormality interferes with regulation of calcium in the muscle. An abnormal ryanodine receptor that controls calcium release causes a buildup of calcium in skeletal muscle, resulting in a massive metabolic reaction.

This hypermetabolism causes increased carbon dioxide production, metabolic and respiratory acidosis, accelerated oxygen consumption, heat production, activation of the sympathetic nervous system, hyperkalemia, disseminated intravascular coagulation (DIC), and multiorgan dysfunction and failure. Early clinical signs of MH include an increase in end-tidal carbon dioxide (even with increasing minute ventilation), tachycardia, muscle rigidity, tachypnea, and hyperkalemia. Later signs include fever, myoglobinuria, and multiorgan failure.

Anesthetics are inconsistent in triggering MH. A susceptible individual may undergo anesthesia with MH-triggering agents on multiple occasions without incident but still react to such agents on a subsequent occasion. A history of uneventful anesthesia with MH-triggering agents does not rule out susceptibility to MH. In fact, there are reports of MH episodes occurring even with the use of "safe" agents.

More than 30 mutations account for human MH. Genetic testing is available to establish a diagnosis, but the caffeine halothane contracture test (CHCT) remains the criterion standard. Dantrolene, the antidote, decreases the loss of calcium from the sarcoplasmic reticulum in the skeletal muscle and restores normal metabolism.Early detection and treatment improve the outcome.
